The online Research Grant Application Form requires answers to various questions concerning the proposed research including the Sponsoring Optometric Institution, qualifications of the applicant, the project plan, objectives/significance/benefits, budgetary account, and publication plan.

Applicant should complete the form with appropriate attachments, including letters of support and/or recommendation from faculty of sponsoring institution.

Quotes for equipment or services are required. Your grant will not be considered until these are received. You can email forms to contactMFVA@gmail.com. If there are price changes after your grant request is approved, your organization is liable for covering the increase in funds.

Grants may only be requested by nonprofit organizations. A copy of your 501c(3) status or IRS determination letter is required before your grant will be considered

Grants will be reviewed by the MFVA Board shortly after the deadline dates of September 1 and March 1 of each year. It is imperative that your request be submitted prior to or by no later than the above dates to be considered by the Board in a timely manner.

A final report is required for all grants. Furthermore, when a grant requests is received from a group or individual who has already received a grant and not submitted a final report, the new grant will not be considered until past grant final reports are received.

Please submit any receipts for equipment or services that have been received. In your final report any remaining receipts that haven't already been submitted must be provided at that time. All receipts and invoices can be emailed to contactMFVA@gmail.com.

All grants are expected to be completed within 2 years of when the grant is approved. If your project will go past that deadline, please fill out a grant request and in that form request an extension on the deadline

MFVA Grant and Scholarship Criteria

PLEASE NOTE that as a potential recipient of a MFVA scholarship, it is understood and agreed that should a scholarship be awarded to the applicant, upon receipt of the applied for scholarship by the applicant, MFVA is granted license by the applicant to use the applicant's name and image/likeness/logo for the limited purpose of publicizing the disbursed scholarship in MFVA publications/social media and/or at MFVA public appearances.  Requests for exemption from the aforementioned licensing agreement must be made separately in writing and accompany the scholarship application at the time of its submission.
